Seventh and eighth-grade students must sign up and complete requirements for at least three of the five modules for the year. Ninth & Tenth-grade students must sign up and complete requirements for at least two of the five modules for the year. There is no minimum requirement for Post Confirmation students. Some modules are restricted to students in certain grade groups.

Post Confirmation Students who are not signing up for any modules must still submit this form
